PUBLIC NOTICE

Caretenders of Jacksonville, LLC d/b/a Apex Home Healthcare, which is located at 4729 U.S. Highway 17, Suite 203, Fleming Island, FL 32003-8244 will close effective October 17, 2022, and will no longer participate in the Medicare Program as provided under Title XVIII of the Social Security Act. The Medicare provider agreement between Caretenders of Jacksonville, LLC and the Federal Department of Health and Hospitals will terminate effective October 17, 2022. The Medicare program will not make payment for services furnished to any patients who may be admitted on or after October 17, 2022. However, Medicare payment is available for services provided under a plan of care established before the effective date of termination for a period of thirty (30) days following closure.
