Health Department giving free back-to-school immunizations

GREEN COVE SPRINGS – The Department of Health Clay County will provide immunizations through Aug. 17 at the Clay County Fairgrounds from 8 a.m. to 2 p.m., Monday through Friday.

There is no cost, but there is a $12 charge for DH680 or DH681 forms without a vaccine.

Vaccinations also are offered at the Bear Run Clinic at 3229 Bear Run Blvd. in Orange Park. Call (904) 529-2800 to make an appointment.

The health department provides primary health services, disease screenings, birth certificates and immunization all year.

A natural or adoptive parent, legal custodian or guardian or a person with a power of attorney to provide medical consent for a child must accompany children. Unless immunizations are recorded in Florida SHOTS, parents are required to bring immunization records.
