NOTICE OF PUBLIC HEARING BEFORE THE BOARD OF COUNTY COMMISSIONERS ON APPLICATION FOR A CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY FOR AMBULANCE SERVICE

The Clay County Department of Public Safety has received an application from Orange Park Medical Center, Inc., d/b/a HCA Florida Orange Park Hospital, for a Class B Certificate of Public Convenience and Necessity (COPCN) and is in the process of reviewing it.
Pursuant to Clay County Ordinance 2018-23, as amended, codified at Chapter 7.3 of the Clay County Code, notice is hereby given that a public hearing before the Board of County Commissioners will be held on Tuesday, April 23, 2024, at 5:00 p.m., or as soon thereafter as can be heard, to consider the application. The public hearing will be held at the following location:

477 Houston Street
Green Cove Springs, Florida
All interested persons are invited to attend the public hearing and be heard on matters pertaining to the application. Pursuant to Section 286.0105, Florida Statutes, a person deciding to appeal any decision made by the Board of County Commissioners with respect to any matter considered at the hearing or at any subsequent hearing to which the Board has continued its deliberations is advised that such person will need a record of all proceedings and may need to ensure that a verbatim record of all proceedings is made, which must include the testimony and evidence upon which the appeal is to be based. A copy of the application is available for review during normal business hours at the Clay County Department of Public Safety, 2519 State Road 16 W., Green Cove Springs, Florida, Telephone number: (904) 284-7703, between the hours of 8:30 a.m. and 4:30 p.m., Monday through Friday, with the exception of legal holidays.
