MIDDLEBURG – Palms Medical Group will be opening a new primary care office in Middleburg on Thursday, Oct. 29. The office will be located at 2620 Blanding Blvd., and its hours of operation will be Monday through Friday, from 8 a.m. to 5 p.m.

Primary and preventive care services will be offered at this location, along with pediatrics, in-house lab draws and access to telehealth providers for established patients (including Behavioral Health services.)

Palms Medical Group is a long-standing, not-for-profit primary care facility established in 1971. The organization accepts most commercial insurance plans, as well as Medicare, Medicaid and uninsured/underinsured patients. Palms’ Mission is to enhance the health and well-being of its communities, which is carried out through its vision of being a leader in health care through an intense focus on caring for the whole person.

Palms operates on a bedrock of six core values: compassion, accountability, initiative, excellence, loyalty and trust.

“We are excited to expand Palms Medical Group’s services to Middleburg and further strengthen access to high-quality health care in Clay County,” said Anita Rembert, CEO of Palms Medical Group. “This new location reflects our commitment to enhancing the health and well-being of the communities we serve through compassionate, patient-centered, whole-person care.

The Middleburg office will feature more than 6,500 square feet, including 15 examination rooms, multiple provider offices, a comfortable lobby, and other patient and staff amenities. This office will join 13 other Palms Medical Group locations in North Central Florida, spanning Alachua, Bradford, Clay, Columbia, Gilchrist, Levy, and Suwannee Counties.

“At Palms, we believe every individual and family deserves access to quality health care close to home, and we are honored to bring our dedicated team and services to the Middleburg community,” Rembert said. “We look forward to building lasting relationships, earning the trust of our patients, and serving as a partner in helping our neighbors lead healthier lives.”

An official grand opening and ribbon cutting ceremony will occur in the months following the office opening. Follow Palms Medical Group on social media for additional information about the ceremony once it is available.
